As controversy persists around the world regarding the effectiveness and safety of vaccines, as well as ongoing discussions about the usefulness of vaccination in general, or in specific populations, at the end of December 2016. Medscape posted an article on the five most common misconceptions and fears about vaccines.

Myth 1: Vaccination is no longer needed

The belief that vaccination is no longer necessary is based on the misconception that most of the diseases we are vaccinated against have disappeared. Indeed, it is true that many diseases that were very common in the past (and some of them, such as diphtheria and polio, were associated with significant mortality or long-term disability of the sick), are now rare in developed countries, so rare that ordinary people and even some medical professionals may believe they no longer exist. However, this is not true. In fact, the only disease that mankind has officially managed to get rid of is smallpox, the last case of which was reported in Somalia in 1977. Of course, vaccination has been able to achieve a significant reduction in the incidence of several infectious diseases, such as tetanus, diphtheria, whooping cough, measles, mumps, polio and congenital rubella.

Nevertheless, the example of measles can very clearly show the importance of mass vaccination in preventing serious diseases. Measles is a highly contagious viral disease that can be accompanied by severe complications. She met very often in the United States until, in 1963, vaccination was not started: at that time, about 4 million cases of measles were registered annually, which was associated with approximately 450 deaths per year. In 2000 it was announced that there was no longer endemic measles in the United States, but imported cases were still reported from travelers and migrants. In 2015 A total of 159 cases of measles have been reported in the United States. Most of these patients either did not receive the vaccine (45%) or their vaccination status was unknown (38%).

The growing number of people who choose not to be vaccinated may create problems for the rest of the population as well, as it leads to a weakening of "herd immunity". This fact is often underestimated by both the public and scientists, but herd immunity protects vulnerable populations who cannot receive full vaccination (for example, patients with immunodeficiency conditions) from potentially serious and life-threatening diseases by reducing the likelihood of transmitting infectious agents to them from other members. society.

Myth 2: Vaccines cause autism

This myth is a very common argument against vaccination. It is based mainly on the publication in 1998. Lancet article by Andrew Wakefield et al. In this publication, Wakefield suggested an association between measles, mumps and rubella vaccine administration and autism based on eight clinical cases, and this raised significant vaccine safety concerns. However, a number of important shortcomings were found in this work during a detailed investigation, including significant methodological flaws and conflicts of interest. These problems led the Lancet to withdraw this publication - partly in 2004. and completely in 2010. Moreover, the main author of the article was deprived of his license to practice medicine by the General Medical Council of Great Britain, because his professional conduct was found to be unfair.

However, these suspicions were initially taken very seriously by the medical community and subsequently several high methodological epidemiological studies were performed to evaluate the possible association between vaccines and autism. Moreover, a meta-analysis of data from five cohort studies (comprising a total of 1,256,407 children) and five case-control studies (involving 9920 children) on the association between vaccines and the development of autism or autism spectrum disorders was subsequently published. spectrum. No associations were found between measles, mumps, and rubella (MMR) vaccination and autism (odds ratio [OR], 0.84; 95% confidence interval [CI], 0.70–1.01).

Separately, the effects of two components of vaccines (thimerosal and mercury), which were accused of causing autism, were studied. No associations with autism were found for either thimerosal (OR, 1.00; 95% CI, 0.77–1.31) or mercury (OR, 1.00; 95% CI, 0.93–1 .07).

In addition to the fact that the association between the MMR vaccine and autism can be safely considered refuted, it should be kept in mind that a significant burden of morbidity and mortality from potentially preventable infectious diseases is well proven and real, including deaths in developed countries: for example, in 2015 in Spain, a 6-year-old boy died of diphtheria, and in 2016. in Belgium, a 3-year-old girl, and both of these children were not vaccinated.

Myth 3: Vaccines cause autoimmune diseases

The role of vaccines in the pathogenesis of autoimmune diseases (presumably as trigger factors that trigger the autoimmune process) has been discussed for quite some time. Although the cause of these diseases is still unclear, it is clear that several factors may play a role, including genetic predisposition, certain environmental characteristics, and infectious diseases.

The relationship between vaccines and autoimmune diseases is still being studied; however, at the moment, no sufficiently convincing evidence of the existence of a causal relationship between them has been obtained. Much of the data on the association between vaccines and autoimmune diseases has come from case reports, which are considered very low levels of evidence. So far, large epidemiological studies that could provide us with convincing clinical information on this issue have not been conducted. Given the characteristics of autoimmune diseases and their diversity, such studies are very difficult.

Recently, the concept of autoimmune/autoinflammatory syndrome induced by adjuvants (ASIA) has been put forward. Several new autoimmune conditions with a potential association with adjuvants (substances used to increase the immunogenicity of a vaccine) have been proposed to fall into this category. ASIA has received considerable attention from the scientific community and many articles have been published on the subject. However, for now, it remains more of a theoretical concept, with very general criteria and insufficiently clear evidence.

There are clinical studies that have compared the incidence of autoimmune diseases in vaccinated versus unvaccinated groups. None of them showed an increase in the incidence of any autoimmune disease in the case of vaccination. The authors of the Medscape article take the view that, given the undeniable benefits of vaccination, this theoretical risk should not be an obstacle to it.

Myth 4: Influenza is a harmless disease, so vaccination is optional

Although the flu is generally considered a mild illness, this is not true for all cases. Influenza poses a significant threat to public health. In the 20th century alone, there have been three pandemics, and millions of people have died from the flu. During the last H1N1 virus pandemic (June 11, 2009 to August 1, 2010), 18,449 deaths were associated with laboratory-confirmed influenza according to WHO, although the overall mortality rate was certainly higher.

Influenza can cause serious complications, including severe pneumonia, as well as effects on other organs, such as encephalopathy and myocarditis. In addition, influenza epidemics are always followed by a significant number of deaths from its cardiac and pulmonary complications. The risk of influenza complications is especially high in the elderly, those with chronic illnesses, and pregnant women, so influenza vaccination is strongly recommended for them.

Myth 5: Vaccines should not be given to pregnant women

Most vaccines are not only safe during pregnancy, they are recommended during this period. Two vaccines are especially important for pregnant women: the Tdap combined tetanus, diphtheria and pertussis vaccine (acellular vaccine), which should preferably be given between 27 and 36 weeks of gestation, and the influenza vaccine. Tetanus, whooping cough and influenza are diseases with potentially severe consequences for the baby and/or mother and can be prevented by vaccination. It has been proven that vaccination of a pregnant woman against whooping cough creates significant protection against this infection for the newborn.

An analysis of the available data suggests that vaccines based on inactivated microorganisms can be safely used at any stage of pregnancy. This is especially true for influenza, which can be very severe in pregnant women, and therefore seasonal influenza vaccination is recommended for pregnant women. The safety of influenza vaccination in pregnant women has been studied in a number of studies, and a meta-analysis found no association between influenza vaccination in any trimester and congenital malformations and malformations (OR, 0.96; 95% CI, 0.86–1 .07). There are also studies confirming the safety of use during pregnancy for vaccines for the prevention of hepatitis B, pneumococcal polysaccharide vaccine and meningococcal polysaccharide vaccine.

Conversely, live virus vaccines, such as varicella or herpes zoster, and MMR are not recommended within 1 month before conception and throughout pregnancy due to the potential risk of virus transmission to the fetus. . At the same time, there is evidence from retrospective studies involving women who received vaccine preparations containing live viruses during pregnancy, and these studies did not show an increase in the risk of congenital infections. However, the period of pregnancy remains a contraindication for the use of such vaccines.

Main conclusions

Currently, vaccination remains the subject of a number of myths and speculations. Even though the scientific basis for these fears looks rather dubious, they have become a real threat to mass vaccination programs. Unfortunately, as a result of these fears and false information, quite a few people refuse vaccinations, both for themselves and for their children. Although there is a need for continued scientific research in the field of vaccination, aimed at both obtaining more effective vaccine preparations in the future (for example, for the prevention of influenza) and studying the immunogenicity of vaccines, it is very important that the entire medical community now supports vaccination and helped maintain adherence to immunization programs.

There are a huge number of diseases of the endocrine system, but autoimmune thyroiditis in children is insidious. The symptoms of this disease do not manifest themselves for a long time, and in the meantime, the body is destroying thyroid cells. In a neglected case, a constant intake of hormonal drugs is required to continue a normal life.

Causes of the disease

By itself, the disease cannot appear. Thanks to many studies, it has been found that the likelihood of developing autoimmune thyroiditis (AIT) depends on the genetic predisposition. If there were cases of the disease in the pedigree, then the chance to shift them to the child is very high.

Push to the development of AIT can:

  • vaccination (in particular against diphtheria, influenza and tetanus);
  • bad ecology;
  • stress and nervous environment;
  • infection with viruses or bacteria;
  • hormonal changes.

How clearly the disease manifests itself depends on the age and gender of the person. It appears much more often in girls than in boys. The peak incidence is considered to be after six years of age.

Processes in the body

With poor heredity or after stress, the mechanisms of immunity weaken. During this period, foreign bodies enter the body and begin their destructive action. At the same time, a hormonal failure occurs, and the immune system begins to work against itself, producing antibodies, the action of which is directed to harm the body.

A similar process directly affects the thyroid gland, destroying healthy cells.

Clinical picture

The asymptomatic course of the disease discourages many parents. Autoimmune thyroiditis in a child does not easily manifest itself for several years, and its presence becomes clear only when examining the thyroid gland.

But there are a number of symptoms to watch out for:

  • enlargement of the thyroid gland - while the child's neck also changes in size. Accompanying symptoms are pain and discomfort. The gland becomes more solid;
  • dysphagia appears - a violation of the swallowing function;
  • breathing is difficult and shortness of breath periodically appears;
  • in the morning there is great dryness in the mouth, and there is no thirst.

And hormonal disorders can also provoke diseases. At the very beginning of the disease, thyrotoxicosis is typical. You can recognize it by the following features:

  • anxiety and worry for no reason;
  • capriciousness and tendency to tears;
  • nightmares;
  • weight loss, accompanied by an increase in appetite;
  • tachycardia;
  • increased sweating.

In the further course of the disease, the level of hormones decreases, this leads to deficiencies in the functions of the thyroid gland. The child has the following symptoms:

  • weight gain due to metabolic disorders;
  • depressed, drowsy state;
  • problems with concentration and memory;
  • hypotension.

The child's reactions are inhibited, the face becomes pale and an unhealthy blush appears on it. Hair becomes weak and brittle, prone to falling out. Sometimes entire bald patches are visible on the head. Adolescents experience hair loss in the armpits and pubis.

To determine the cause of such clinical manifestations, it is necessary to undergo an additional examination.

Diagnostics

Accurately diagnosing autoimmune thyroiditis is realistic only after, but such a study is practically not carried out for children. But they examine the blood for the level of thyroid hormones - T3 or T4, as well as TSH. They also take an analysis for antibodies to thyroglobulin or peroxidase.

Sometimes an ultrasound is performed, during which the structure of the thyroid gland is studied.

Complications

If you do not start taking any measures in time, everything can end very sadly.

Serious deviations will occur in the child's body. Children suffering from thyroiditis lag behind their peers in development. As the disease progresses, decreased production of thyroid hormones leads to hypothyroidism. The increase in lipid levels that occurs with the disease contributes to the development of coronary heart disease.

Sometimes thyroid lymphomas appear. The reasons are not yet known, but it does happen. It is characterized by rapid growth, despite the right treatment.

Oncology due to thyroiditis does not appear, but these diseases can peacefully coexist in one person.

The functionality of the body will suffer if the thyroid gland is unable to do its job. Weakened immunity is unable to protect against the penetration of bacteria and infections, moreover, due to illness, it will produce antibodies against itself, which will contribute to the worsening of the situation.

Treatment

Specific therapy for thyroiditis for children has not yet been developed. Symptomatic remedies are the first helpers in this disease.

When hypothyroidism is detected, treatment is reduced to the use of thyroid hormones. These include medicines that contain L-thyroxine. Usually it is Levothyroxine. During the treatment period, the clinical picture and the level of thyrotropin in the blood serum are under control. But drugs only slow down the development and do not protect against the progression of the disease in the future.

An increase in the functionality of the thyroid gland is very rarely provoked by autoimmune thyroiditis. But if this happens, thyrostatics are prescribed. These include "Tiamazol" and "Mercazolil".

Glucocorticoids are used if there is a combination of the autoimmune phase of the disease with. Most often this happens in the cold season.

Anti-inflammatory therapy is carried out to reduce the production of antibodies. In this they have proven themselves well: "Indomethacin" and "Metindol".

Restrictions

There are a number of restrictions that must be followed in a family where there is a child diagnosed with autoimmune thyroiditis:

But the whole incident is that such drugs can both help and harm. With autoimmune thyroiditis, iodine increases the number of antibodies that destroy the thyroid gland.   • selenium - the development of hypothyroidism can provoke selenium deficiency. This microelement plays an important role in the disease. But not in all cases, its appointment is recommended: a contraindication is the presence of thyrotoxicosis;
  • vaccines - often parents are concerned about the vaccination of children with impaired thyroid function. Experts are sure: vaccinations and autoimmune thyroiditis are incompatible concepts. Vaccination can only exacerbate the hormonal imbalance present when immunity is impaired during illness.

With timely treatment of the child, everything will end well. To prevent such a disease in the future, it is necessary to control hormone levels. Periods of exacerbation may remind you of pathology, but following all the recommendations will give a lasting positive effect.

So, we talked about general contraindications to vaccinations () and got to the question of special conditions. In which vaccinations may be delayed or even prohibited. It is necessary to discuss in detail all the particular issues of contraindications and restrictions on vaccinations.

If immunodeficiency.
The vaccination process is an immune reaction of the body with the active work of all parts of the immune system, therefore, in order for it to go relatively smoothly, it is necessary that the immune system be healthy and work adequately. This is not always the case. Sometimes there are states of immunodeficiency, temporary or permanent impairment of immunity with inadequate or reduced reactions to seemingly common stimuli - diseases, nutrients and drugs. Therefore, in the instructions for contraindications, the issue of immunodeficiency is considered separately.

First of all, this is due to the fact that in children with immunodeficiencies, the risk of vaccination with live vaccines increases. Although they are weakened, but purely theoretically they can be harmful to severely undermined immunity. Such complications include vaccine-associated polio when instilled with live polio vaccine. In addition, BCG and the vaccine against rubella, partitis and measles will be dangerous for such babies. But how to suspect the presence of immunodeficiency, because universal testing and immunograms are not done? There are certain health issues that will allow you and your doctor to think about problems in the immune system. These are, first of all, severe, recurrent purulent processes, the development of anorectal fistulas, persistent oral thrush, frequent pneumonia, persistent eczema, seborrhea, a decrease in blood platelets, the presence of relatives with immunity problems. Such children are replaced with dead vaccines, and they are examined, but BCG is not carried out for such children.

Children who have recently been treated with immunosuppressive drugs should not be vaccinated if they are being treated with hormones, especially by mouth or by inhalation. It should be at least three to six months after taking these drugs and getting vaccinated.

General asthenia does not belong to the signs of immunodeficiency, this is when the “child is weak”, frequent colds, long-term recovery. Such children after recovery or in remission are vaccinated in the general manner. It would be illegal for a doctor to refuse to vaccinate a child (but parents decide here, they can write a refusal). When not to do BCG?
BCG is a weakened live Mycobacterium tuberculosis, they give a local intradermal process. Babies weighing less than 2 kg at birth should not be vaccinated. These vaccines should not be administered if children have previously had reactions to antibiotic treatment such as gentamicin, amikacin, and the like. The vaccine contains traces of these drugs. The imported MMR vaccine is prepared on chicken embryos and can be dangerous for children who are allergic to chicken protein. Patriotic is prepared on a quail egg and is not dangerous for such babies.

When not to do hepatitis B?
We have already said that this vaccine should not be given if there is an allergy to yeast, and if there are indications that there is a history of multiple sclerosis in the family.

If the baby has an acute infection?

Always in the event of acute infections (intestinal, colds, skin and others), the work of the immune system changes, it directs all its forces to fight the enemy and an additional load on the body can fail and negatively react. Therefore, under normal conditions, when there are no deadly threats of epidemics of bubonic plague or deadly fever, children are not vaccinated with anything until they recover from an acute illness. On average, with a common cold or a mild intestinal infection, after a complete recovery, they give a medical exemption from vaccinations for two weeks. And if it was a sore throat, bronchitis - then for a month, and with pneumonia it can be longer.
If these were severe acute infections of the nervous system, such as encephalitis and meningitis, children are generally vaccinated only after six months.

If the baby has a chronic illness?
Also, as with acute infections and diseases, children with exacerbations of any chronic disease cannot be vaccinated with any vaccines, even droplets from polio. It is possible to vaccinate a child against the background of a remission that occurred independently or after treatment, or by reaching the minimum possible activity of the process and having previously given the child special training. If immunosuppressive therapy or hormones are used to achieve remission, then vaccination is also canceled, immunity may give an inadequate response.

For such children, vaccination is carried out according to special schemes, they have their own, individual calendar, developed by a pediatrician or immunologist. Vaccination is carried out against the background of "cover" drugs or basic therapy.

What is not a contraindication?
There are a number of special conditions that some doctors mistakenly take as contraindications, and patients stubbornly regard them as an obstacle to vaccination. They are called false contraindications, which cannot be the reason for rejection.

On average, a real medical withdrawal from all or many vaccinations is found in about 1-2% of children, in all the rest the reasons for the medical withdrawal are “socially significant”, and not real. Often having a flowery diagnosis of “pep”, encephalopathy, dysdacteriosis or anemia, rickets, allergies, dermatitis in the card, children also have a medical exemption from vaccinations, and almost for life. From the point of view of medicine, this position is incorrect, since some of these diagnoses have no real power, except for the horror of parents from terribly incomprehensible words.

- asthma with allergies, eczema,

It is a receipt for its own incompetence in matters of immunoprophylaxis. No, of course, such conditions require a special approach in matters of vaccination, but from a medical point of view, it is impossible to completely reject it, it is necessary to develop a flexible and correct vaccination scheme.

I already wrote about the term pep in my materials earlier and I won’t dwell on it in detail, the diagnosis says that “the child has something with his head” (and you need to read it from the doctor ....). Accordingly, such a diagnosis and withdrawal from vaccinations cannot be. We have also already discussed the presence of anemia; in children aged from 3 to about six months, it is generally a physiological phenomenon. In addition, the number of red blood cells does not affect the immune system in any way - it is not erythrocytes that are responsible for it, but lymphocytes. Therefore, mild anemia is not a reason to refuse vaccinations. And severe anemia is an occasion for examination, it usually has serious causes.

I don’t even want to talk about dysbacteriosis, this word is like a curse word for me, the one who leaves him with a diagnosis is a loser and a truant at a medical university! Normal doctors know that this answer is when you don’t know what to do and how!

With allergies and heart defects, children are weakened, and vaccination infections in them are much more difficult than in ordinary healthy children. And we recommend that such children be vaccinated according to a reasonable individual scheme, but first of all. After all, bouts of whooping cough for a baby with heart disease can be fatal. Acquired immunity occurs after the transfer of diseases and persists for a long time or for life.

Artificial immunity and vaccines
